I was going for either equipment failure and they dropped into the sun; insanity; or a little of both. (Darn I just used your exact wording for those )

They made that wrong turn at Alberqurque and ended up near the Sun. After that they ran into Marvin the Martian who vaporized them or his dog chased them away (Bugs Bunny reference).

I'd say the team couldn't work together well causing major stress, therefore turning every person insane...

you know u can get crazy up there. imagine you're millions of miles away from your own planet heading to the sun and you have a plan that has a 40% chance of success....hmmm.... the good thing is that they have developed artificial gravity...imagine wat would have happened 2 them had they not had gravity..just the simple tasks like eating or getting dressed (you couldn't even shower in zero grav.) would be arduous and even dangerous. not to mention your ship's carrying a bomb the size of Manhattan....i wouldn't be surprised if sum1 got crazy on that kind of trip. They should have sent Icarus II in the beginning and avoid the darn problems the characters face in the movie...then again movies are sumtimes made to frustrate us..
